Please print the above “New Patient” form and fill it out. We ask that you bring the completed form with you to your scheduled appointment. You can also fax to 256-667-4145 if you prefer. If you choose to email the form to us, please understand that we cannot guarantee the security of your information, but we will be happy to accept the email if you prefer this method. Once we receive your form, we will call you and set up your first visit with Southern Grace Medical Care if one has not already been made. If something is preventing you from bringing or sending the form before your first visit, or if you need to be seen immediately, please call us and we will be happy to work with you to get your information, so we can accommodate you. Please print and fill out the above “History and Authorization” form and send it in with your demographic form. This will keep you from having to fill these out when you get here. You may fill it out and bring it to your first appointment if you have any issues sending it in. If you choose this option, we ask you to arrive 30 minutes early to your first appointment, so that we can enter the information before your actual appointment time.
